Analyzing failures can provide many lessons. For this purpose, it's crucial to first have an attitude of learning from failures and to establish a culture within the organization where failures are not met with blame.
In the event of an incident, it may be necessary to stop all operations to conduct an analysis, if deemed important.
I believe that it is better to conduct analyses of failures and provide feedback sooner rather than later. "Feedback that is delayed lacks effectiveness in enhancing intuitive judgment," as stated in this book. What's important here is the realization from experience that feedback is valuable in pointing out mistakes and helping to improve skills and the environment.
In fact, I made a mistake yesterday. Reading this book, I want to have the mindset of not turning away from failures.
