What I Learned from Chapter 5 of '失敗の科学'.
Our brains tend to jump to the simplest and most intuitive conclusions. However, in reality, mistakes often arise from complex factors. In such cases, merely increasing penalties will not reduce these mistakes; it will only decrease the reporting of them. Consequently, mistakes become deeply buried, creating a culture of concealment. To build an open organizational culture that encourages the reporting of failures, the first step is to stop premature blame. Philosopher Karl Popper said,
"True ignorance is not the absence of knowledge, but the refusal to learn."
An organization that reflexively assigns blame and penalties without attempting to learn from failures embodies true ignorance.
